Planning for end of life can be a challenging task, but you can have peace of mind in knowing you have communicated your medical preferences to your family, caregivers, and healthcare professionals by making an Advance Directive, a legal document that expresses your decisions about end-of-life care.
It’s a difficult but necessary discussion about:
-Who you want to make healthcare decisions for you when you are unable to
-The kind of medical treatment you want or don’t want
-How comfortable you want to be
To help get you started, Civista will hold an Advance Directives lecture on November 10, from 6:30 p.m. to 7:30 p.m. in the Nagula Conference Room. The lecture is free and open to the public as part of Civista Health’s Community Education program. Each participant will receive a free “5 Wishes” advanced directive document.
